Do tennis officials ever get the decisions wrong in the game so that the challenge system actually works?

yes, but not very often from my experience

I've won a couple challenges but more often than not I'm wrong maybe an increase in errors from the linesmen is in order. Also maybe a way to increase the time limit of a challenge without it effecting gameplay.
